Poster 284: Investigating the Frequency and Extent of Articular Cartilage Injury Following Patellar Dislocation: A Comparison of Recurrent vs. First-Time Dislocations

Objectives: In cases of patellar dislocation, surgical intervention is typically reserved for patients with recurrent instability, unless a chondral or osteochondral fragment identified following the initial dislocation. There concern that multiple dislocation events may result in higher incidence articular cartilage injury. The purpose this study to compare damage single those dislocations, and secondarily location severity injury between groups. Methods: Patients diagnosed were retrospectively by ICD-9 & ICD-10 codes from 2017-2021. Inclusion criteria documented history an MRI affected knee available review. prior event excluded. Medical records reviewed determine number previous dislocations. Articular was graded using validated system, AMADEUS (mean total Area Measurement And DEpth Underlying Structures, scale 0 100; 100 = no injury). Parametric continuous data calculated performing t tests. Nonparametric Mann-Whitney Chi-Square Fisher’s Exact tests used calculate p values categorical data. Results: After chart imaging review, 233 included: 117 primary 116 Fifty-one (43.6%) dislocations 68 (58.6%) found have injuries (p=0.068). When comparing groups, there difference age, BMI, race, laterality, but group contained significantly larger proportion females (46.2% vs. 65.5%, P=0.004). lesion size, subchondral bone defect, presence edema, score groups (p=0.231). Subgroup analysis revealed when present, had more full-thickness lesions (p<0.001) lower scores (p=0.016). increased medial facet lateral femoral condyle (p=0.026).
